Hemolytic Disease of the Newborn ————————————————- What is hemolytic disease of the newborn (HDN)? Hemolytic disease of the newborn is also called erythroblastosis fetalis. This condition occurs when there is an incompatibility between the blood types of the mother and baby. * â€Å"Hemolytic† means breaking down of red blood cells * â€Å"Erythroblastosis† refers to making of immature red blood cells * â€Å"Fetalis† refers to fetus ————————————————- What causes hemolytic disease of the newborn (HDN)?HDN most frequently occurs when an Rh negative mother has a baby with an Rh positive father. When the baby's Rh factor is positive, like the father's, problems can develop if the baby's red blood cells cross to the Rh negative mother. This usuall y happens at delivery when the placenta detaches. However, it may also happen anytime blood cells of the two circulations mix, such as during a miscarriage or abortion, with a fall, or during an invasive prenatal testing procedure (e. g. , an amniocentesis or chorionic villus sampling). The mother's immune system sees the baby's Rh positive red blood cells as â€Å"foreign. Just as when bacteria invade the body, the immune system responds by developing antibodies to fight and destroy these foreign cells. The mother's immune system then keeps the antibodies in case the foreign cells appear again, even in a future pregnancy. The mother is now â€Å"Rh sensitized. † In a first pregnancy, Rh sensitization is not likely. Usually, it only becomes a problem in a future pregnancy with another Rh positive baby. During that pregnancy, the mother's antibodies cross the placenta to fight the Rh positive cells in the baby's body.As the antibodies destroy the red blood cells, the baby can become sick. This is called erythroblastosis fetalis during pregnancy. In the newborn, the condition is called hemolytic disease of the newborn. ————————————————- Who is affected by hemolytic disease of the newborn? Babies affected by HDN are usually in a mother's second or higher pregnancy, after she has become sensitized with a first baby. HDN due to Rh incompatibility is about three times more likely in Caucasian babies than African-American babies. ———————————————— Why is hemolytic disease of the newborn a concern? When the mother's antibodies attack the red blood cells, they are broken down and destroyed (hemolysis). This makes the baby anemic. Anemia is dangerous because it limits the ability of the blood to carry oxygen to the baby's organs and tissues. A s a result: * The baby's body responds to the hemolysis by trying to make more red blood cells very quickly in the bone marrow and the liver and spleen. This causes these organs to get bigger.The new red blood cells, called erythroblasts, are often immature and are not able to do the work of mature red blood cells. * As the red blood cells break down, a substance called bilirubin is formed. Babies are not easily able to get rid of the bilirubin and it can build up in the blood and other tissues and fluids of the baby's body. This is called hyperbilirubinemia. Because bilirubin has a pigment or coloring, it causes a yellowing of the baby's skin and tissues. This is called jaundice. Complications of hemolytic disease of the newborn can range from mild to severe.The following are some of the problems that can result: During pregnancy: * Mild anemia, hyperbilirubinemia, and jaundice The placenta helps rid some of the bilirubin, but not all. * Severe anemia with enlargement of the liver and spleen   When these organs and the bone marrow cannot compensate for the fast destruction of red blood cells, severe anemia results and other organs are affected. * Hydrops fetalis This occurs as the baby's organs are unable to handle the anemia. The heart begins to fail and large amounts of fluid build up in the baby's tissues and organs.A fetus with hydrops is at great risk of being stillborn. After birth: * Severe hyperbilirubinemia and jaundice The baby's liver is unable to handle the large amount of bilirubin that results from red blood cell breakdown. The baby's liver is enlarged and anemia continues. * Kernicterus Kernicterus is the most severe form of hyperbilirubinemia and results from the buildup of bilirubin in the brain. This can cause seizures, brain damage, deafness, and death. ————————————————- What are the symptoms of hemolytic disease of the newborn? The following are the most common symptoms of hemolytic disease of the newborn. However, each baby may experience symptoms differently. During pregnancy symptoms may include: * With amniocentesis, the amniotic fluid may have a yellow coloring and contain bilirubin. * Ultrasound of the fetus shows enlarged liver, spleen, or heart and fluid buildup in the fetus's abdomen. After birth, symptoms may include: * A pale coloring may be evident, due to anemia. * Jaundice, or yellow coloring of amniotic fluid, umbilical cord, skin, and eyes may be present.The baby may not look yellow immediately after birth, but jaundice can develop quickly, usually within 24 to 36 hours. * The newborn may have an enlarged liver and spleen. * Babies with hydrops fetalis have severe edema (swelling) of the entire body and are extremely pale. Because anemia, hyperbilirubinemia, and hydrops fetalis can occur with other diseases and conditions, the accurate diagnosis of HDN depends on determining if there is a blood group or blood type incompatibility.Sometimes, the diagnosis can be made during pregnancy based on information from the following tests: * Testing for the presence of Rh positive antibodies in the mother's blood * Ultrasound – to detect organ enlargement or fluid buildup in the fetus. Ultrasound is a diagnostic imaging technique which uses high-frequency sound waves and a computer to create images of blood vessels, tissues, and organs. Ultrasound is used to view internal organs as they function, and to assess blood flow through various vessels. * Amniocentesis – to measure the amount of bilirubin in the amniotic fluid.Amniocentesis is a test performed to determine chromosomal and genetic disorders and certain birth defects. The test involves inserti ng a needle through the abdominal and uterine wall into the amniotic sac to retrieve a sample of amniotic fluid. * Sampling of some of the blood from the fetal umbilical cord during pregnancy to check for antibodies, bilirubin, and anemia in the fetus. Once a baby is born, diagnostic tests for HDN may include the following: * Testing of the baby's umbilical cord blood for blood group, Rh factor, red blood cell count, and antibodies * Testing of the baby's blood for bilirubin levels ———————————————— Treatment for hemolytic disease of the newborn Once HDN is diagnosed, treatment may be needed. Specific treatment for hemolytic disease of the newborn will be determined by your baby's physician based on: * Your baby's gestational age, overall health, and medical history * Extent of the disease * Your baby's tolerance for specific medications, procedures, or therapies * Expectat ions for the course of the disease * Your opinion or preference During pregnancy, treatment for HDN may include: Intrauterine blood transfusion of red blood cells into the baby's circulation   This is done by placing a needle through the mother's uterus and into the abdominal cavity of the fetus or directly into the vein in the umbilical cord. It may be necessary to give a sedative medication to keep the baby from moving. Intrauterine transfusions may need to be repeated. * Early delivery if the fetus develops complications If the fetus has mature lungs, labor and delivery may be induced to prevent worsening of HDN. After birth, treatment may include: * Blood transfusions  (for severe anemia) Intravenous fluids  (for low blood pressure) * Help for respiratory distress using oxygen or a mechanical breathing machine * Exchange transfusion to replace the baby's damaged blood with fresh blood   The exchange transfusion helps increase the red blood cell count and lower the levels of bilirubin. An exchange transfusion is done by alternating giving and withdrawing blood in small amounts through a vein or artery. This indicates the idea of a sound business decision, as Coffee Republic is almost guaranteed to get a lot of customers, as the affluent are a of Finchley Central doesnt have a proper coffee shop; its an untapped market, which will mean that it will be easy to wipe out any probable competition. Arguments against locating in Finchley Central:-Â  Weaknesses:Â  An argument against, which is a weakness, is the fact that the credit crunch has cut peoples spending on indulgences such as what Coffee Republic has to offer. This is best reflected in the interview which I conducted, [refer to interview], where a owner gave us his first hand view of what has happened to its sales in this economic dilemma. The owner stated that recently, maybe as a result of the credit crunch, its very quiet. Instead of buying two cups of coffee with a snack or meal they are buying a single. This shows that the credit crunch may have hit hard on the Finchley Central population, as the owner stated that its much quieter since the credit crunch has happened. A counter-argument which can solve this issue is something that another big branded coffee franchise has initiated. Starbucks has issued free refills on their coffee due to the credit crunch. This could be something that Coffee Republic can initiate if they want to beat the credit crunch, and retain their customers. This would not be a sound business decision, as the people who are affected by the credit crunch may not want to pay the high price for the quality products that Coffee Republic provide, so they may not have as many customers as they would want. Recommendation:- Finchley Central is a well off area brimming with a lot of potential and in my opinion, a sound business decision to set up a new Coffee Republic franchise there. It is an area with a very affluent community and has great transport links making the accessibility high; because it is close to the motorways, it can ensure that efficient deliveries are made. Taking into account all the research which I have done, I would conclude that Finchley Central is a very good location to set up a Coffee Republic, however there are many potential exact locations where it can be set up in Finchley Central. By looking at my secondary data from the West Finchley Census, I can see that the majority of residents in Finchley Central, travel to work using the underground [Graph 7]. Consequently, the location of a Coffee Republic inside Finchley Central underground tube station could be a sound business idea. By locating in the station, there is a possibility of applying for a government grant. Due to the credit crunch and economic downturn, creating jobs is something that the government would want big companies like Coffee Republic to do; this would increase the chance of Coffee Republic getting a grant. On the other hand, by locating inside the tube station, Coffee Republic would be narrowing down their potential market, as it may not be seen inside the tube station; they wouldnt have a lot of scope for customers. But a counter-argument for this is that, hundreds of people use the underground each day, so its likely that Coffee Republic may pick up some customers, as commuters enter and exit the tube station. The final option is to locate on Station Road which is close to Finchley Central station. In my opinion this where Coffee Republic should locate. The site at the moment is vacant and is situated close to potential customers such as offices, banks etc. The advantage of locating on Station Road is that it is of substantial size. If opened up on Station Road, they wouldnt have to pay money to other company, if they want to lease the building; it also has a large potential customer base. However, it may be more expensive setting up the new franchise on Station Road as opposed to inside the station or in Tescos. This is because this a hotspot as it is situated near to the main road, so there could be potential competition between firms for that spot. Nonetheless, I think that Coffee Republic should locate here due to the potential customer base, probability of joint ventures, and the size of the site.

Security * Security mechanisms: There are several mechanisms to secure any Wi-Fi network each mechanism has its own concept:- 1-SSID: SSID is the short form of Service Set ID. In other word, it can be considered as the name of the network. The purpose of SSID is connecting the clients to the access point. SSID will be broadcasting to be seen by the clients. Each access point has a name. In case if there is more than one access point with the same SSID, it will be shown as one access point which has the strongest signal. In the other side, SSID can be hidden for more security by changing the settings of the broadcast to be unviewed so that the client must know the SSID. Unfortunately, hiding the SSAD does not mean that the SSID is really invisible; there are some programs to find out the hidden SSID such as NetStumbler. 2-WEP: is the short form of Wireless Equivalency Protocol. When the user set up Wi-Fi network, there will be two options of encryption, one is WEP and the other is WAP. WEP provides lower level of protection. It uses 64bit and 128bit encryption by using RC4 logarithm. The perception of human anatomy and its function from the Middle Ages to the Renaissance has changed greatly and diminished the importance of Christianity. The first theory, â€Å"Zodiac Man,† believed that each sign of the zodiac ruled a part of the DOD.A zodiac is a constellation, which is part of the universe that God created. With the importance of Christianity, people in Europe at this time surely did believe that these constellations were controlling the body (Document H). Belgian physician, Andrea Vesuvius, who actually dissected the human body to better explain what actually happened, made another theory about how the human body worked. Critical Analysis and Evaluation of “1984” by George Orwell Essay

George Orwell, whose real name was Eric Blair, was born in Bengal, India, in 1903. When he was eight years old, as it was customary, his mother brought him back to England to be educated. He was sent to a boarding school on the south coast, a school whose students were sons of the upper class. He was allowed in with lower tuition and not being from a wealthy background, he was subject to snobbery of the others at the school. In 1917 he went to Eton on a scholarship and he enjoyed himself, making friends and reading many books. Orwell graduated in 1921 and instead of going on to a university; he joined the Civil Service and went to Burma as a sergeant in the Indian Imperial Police. He served as a policeman in Burma from 1922 until he took leave in 1927 and resigned his post. Orwell now had time to think and he decided to live among working-class people in Paris and among the tramps in England for more than a year. During these years he worked as a schoolteacher and got married. Both he and his wife kept a village pub and a village general store. All of these experiences led up to the basis for his first book, Down and Out in Paris and London, which was an autobiographical work he published in 1933. This was his first book and he used the name George Orwell. He explained later that he took the last name from an English river near where he lived and the first name as being typically English. The latter part of his work was written primarily about different things and places that he had experienced. For example, he wrote, Homage to Catalonia, from his experiences in the Spanish Civil War and A Clergyman’s Daughter, makes use of his teaching experiences. Orwell died at the young age of forty-seven from neglected lung  ailments. In his death he left behind a growing reputation for greatness and the conviction that modern man was inadequate to cope with the demands of his history. It is not apparent that Orwell used any sources in his writing of 1984. If he had, they were not included in my copy of 1984. The Author’s Views I feel that George Orwell wrote 1984 as a warning after years of brooding on the twin menaces of Nazism and Stalinism. He wanted to lay out the basis of what he thought the world could become. By writing this anti-utopian story, I feel he hoped to frighten us so that we might be on our guard. If a certain city knew of a tornado on the rise, they could be prepared. But, if that tornado hit that same town and they were not expecting it, the potential damage could be far greater. Orwell was warning us. The position the author takes on his story seems to be a hopeful one. He hopes that Winston will end up defeating Oceania’s mad system of government and find true love with Julia but he is not writing a love story. He planted his seed (the plot) in some nasty soil. The environment in which he set his story would only allow for Winston to get so far before Big Brother got the best of him. He clearly felt sympathy for Winston but he also wanted to make his point for the book clear. Wintson would have to love Big Brother. 1984: In Review Winston Smith, a physically insignificant man of thirty-nine, is Orwell’s principal protagonist. He works in the Records department of the Ministry of Truth, revising the past as it appears in the newspapers, and he is tired of living under the paternalistic government of Oceania. Waking up every morning in this mad society has turned him slowly into a rebel. He first started writing in a diary, which was looked at with suspicion by the Though Police. Writing would record history as it really occurred and this was exactly what the Big Brother did not want. Big Brother is the name of a person that does not exist. He is the major political figure in Oceania that opposes everything Emmanuel Goldstein is about. If one takes the position that â€Å"the party is correct,† than Big Brother becomes God, President and ultimate friend. Goldstein, on the other hand, preached that Big Brother was wrong and eventually had to flee from Oceania because of his growing hatred. The videotapes with which he spoke against Oceania’s politics and Big Brother were used in the daily Two-Minute Hates. In this period of time, work ceased and everyone gathered around a telescreen, large, flat television screens that pervaded Oceania. Goldstein appeared on the telescreen and everyone was to scream and mock this man, for he was not right. A young woman, Julia, instigates a liaison between herself and Winston. Rebelling against Big Brother, they make love, which Winston believes is a severe thoughtcrime. He felt that sexual relationships accompanying love would result in a loyalty between individuals that is contrary to the desires of the Party. They then rent an apartment and take comfort that O’Brien, a vigorous, intelligent man of mysteriously high rank in the Inner Party, is their friend. O’Brien lets Winston read a seditious book. This book describes the relationships of Oceania with Eastasia and Eurasia. Once O’Brien had enough evidence to convict Winston and Julia, the two were arrested in their rented room. They were imprisoned in the Ministry of Love where O’Brien put Winston through the first two stages of his retraining. Over the nine months of â€Å"rehabilitation† he learned to love O’Brien and they conversed as if friends. The last stage came in the dreaded Room 101, where Winston was made to face what he secretly feared most, rats. He betrays Julia and is subjugated physically, mentally, and emotionally. All that remains is for Winston to love Big Brother. Personal Evaluation of Book and Author George Orwell is an excellent author. Before I was introduced with 1984, I read Animal Farm and this gave me a taste for his writing. In both Animal Farm and 1984 alike, Orwell’s writing was easy to adapt to. His style of writing helped my understanding of the book and made it enjoyable. In 1984, he presented a very interesting scenario similar to Nazi Germany. I believe  that Orwell put much effort into this book and made his story very realistic. As I read along, there was nothing which seemed too out of reach or too unbelievable. His protagonist, Winston Smith, was an ordinary citizen in Oceania by appearance but his thoughts and actions were radical according to Big Brother. Orwell in a sense let me down by letting the worst catch up with Winston and having Big Brother be the final destruction of him. This, though, I feel had to happen to fulfill the authors purpose for writing the story. He wanted to lay out the idea of an anti-utopian society and, I think he hoped by presenting it, that it might not happen. I believe that he fulfilled his purpose in writing the story and that he laid out a warning for the potential dangers of totalitarianism. His book was at first thought to be too radical but the more it was read and reread, he seemed to be right on target. He proposed a very interesting story with endless possibilities. Reading 1984 was a learning experience. I learned the value in speaking out against the majority. I also learned the danger in allowing groups to gain too much power. When I was reading the book, I forgot some political aspects about W.W.II and I went back to a report I had written on that subject. After reviewing the war, it made me realize how well Orwell set his thoughts out through the book and how he formulated an interesting story. Orwell might not have been the most creative of all authors but I learned that this did not matter. He worked hard on such an elaborate plot that it was not necessary to be that creative. This was a fictional piece of literature and therefore I could not agree or disagree with the knowledge I previously knew about the book’s subject. Though, the book speaks of a society that was remarkably similar to Nazi Germany. I recommend this book to anyone. It was well written and thoughtful ;I will always remember its impact on me. The best thing about it was that anyone could pick up this book and get the jest of it. It was not imperative to understand what utopia meant or have any knowledge of W.W.II, Nazi Germany, or the Soviet Union or to understand the book. As I had said before, I had to refresh my history of W.W.II but this did not hinder me from enjoying the book.
